Your physician may be an owner of and/or have interests in Glasgow Medical Center, L.L.C., whose services to the community include the Medical Office Building, Glasgow Medical Aid Unit, Glasgow Ambulatory Surgery Center, Glasgow Medical Aid Unit at Christiana and Glasgow Medical Aid Unit at Middletown.
2. You have the right to choose the provider of your health care services. Therefore, you have the option to use a health care facility other than Glasgow Medical Center.
3. You will not be treated differently by your physician if you choose to
obtain healthcare services at a facility other than Glasgow Medical Center.
